How to Get Started with Evergreen Adult Medicine

If you or someone you know is struggling with an addiction to prescription painkillers or heroin, (a.k.a. opioids), our Suboxone program can help.

Suboxone is a prescription-only medication used to treat opioid dependency. We carefully prescribe this treatment to eligible patients and offer counseling and support services to help in their recovery.

Please call (301) 979-9636 for more information. Our program is available to a limited number of patients at a time; participation will require a brief evaluation.

New Patient Form

If you are a newly accepted applicant, please complete the secure online form. After we receive your online form submission, our staff will be in contact to schedule an appointment.

If we do not accept your insurance, a self-pay option is available at an affordable rate.
